骨格筋の筋量と機能は、運動神経の支配によって制御されており、脱神経は筋萎縮をもたらす。哺乳類ラパマイシン標的タンパク質複合体1(mTORC1)の活性は、脱神経筋でかなり増加しているが、脱神経誘導性筋萎縮におけるその制御的役割はまだ明らかになっていない。骨格筋脱神経後の初期段階では、クラスIIヒストン脱アセチル化酵素と転写因子ミオゲニンが関与する経路が脱神経誘導性筋萎縮を媒介する。われわれは、速筋脱神経後の後期段階においてmTORC1活性化が筋萎縮に寄与すること、ラパマイシンを用いたmTORC1の阻害により脱神経誘導性筋萎縮が軽減されることを見出した。阻害因子TSC1(結節性硬化症複合体1)を遺伝的に欠損させてmTORC1を活性化させると、脱神経誘導性筋萎縮に対するマウスの感受性が亢進し、Aktのキナーゼ活性が抑制されることで、FoxO転写因子の活性化がもたらされ、E3ユビキチンリガーゼatrogin[MAFbx(筋萎縮F-boxタンパク質)としても知られる]およびMuRF1(筋特異的ring finger 1)をコードする遺伝子の発現が増加した。マウスにラパマイシンを投与するとAkt活性が回復したことから、脱神経により誘導されるmTORC1活性の増加がAktのフィードバック阻害を生み出していたことが示唆された。骨格筋において3つのFoxOアイソフォームを遺伝的に欠損させると、筋肥大が誘導され、脱神経後の後期段階におけるE3ユビキチンリガーゼの誘導が消失し、それにより脱神経誘導性筋萎縮が防止された。これらのデータから、同化作用の重要な構成要素であると一般的に考えられているmTORC1が、脱神経後の筋肉異化作用および萎縮の中心であることが明らかになった。このmTORC1-FoxO軸は、神経原性筋萎縮の可能性のある治療標的である。
